There are two options available to you if the IRS refuses to accept your OIC. You can submit your offer again. A new Form 656 is not required unless you receive the first offer within a month.
An IRS letter will give you the amount that is acceptable if your offer is rejected. An IRS copy of any report that lists why the offer was rejected will be sent to you. Ask the IRS for one. If the IRS won’t provide it, you may ask the Freedom of Information Act.

After a disaster, taxpayers might need certain records to prove their loss. The more accurately the loss is estimated, the more loan and grant money there may be available.
Relief from Penalties -- IRS has highlighted reasonable cause assistance that is available to taxpayers who are subject to penalty, failure to file, deposit or pay taxes. If a taxpayer is first subject to one or more tax penalties, they can also receive penalty abatement relief.
Once you've done the calculation, IRS will ask you for your available assets. They also want to see a year or more of your income relative to acceptable expenditures. The IRS can ask you to pay for debts you have taken out or expenses you have incurred that exceed your assets.
In most cases you will also be required to pay $150 as your initial payment.

Low-interest, long-term loans can be obtained for losses that aren’t fully covered under insurance or other recoveries.
Taxpayer Advocate ServiceThe Taxpayer Advocate Service is an independent organization that works within the IRS. They assist taxpayers in resolving problems with IRS and suggest changes to prevent them from happening again.
